At The Hammond Group Home Health Care we seize the opportunity to meet the health care needs of the community we serve by providing high quality health care services to individuals in a compassionate, caring and professional way. We work with the patient's physician and family to implement a plan of treatment and ensure continuity of care from the familiar surroundings of home.
Hammond Group Home Health Care staff is screened and credentialed and has experience providing care for adult, geriatric, head injury, hospice, disability, pediatric and high risk pregnant patients with a variety of ailments. We ensure our homecare service is provided for the entire time it is needed and require that our care representatives are available 24-hours a day, 7 days a week, so assistance is always at your finger tips.
Hammond Group Home Health Care provides personal care, management and/or treatment of the following:

Hammond’s Group Home Health Care Team includes:
Skilled Nurses, RN, LPN
Hammond’s Group Home Health Care provides a full range of nursing services including wound care, IV therapy, diabetic care, cardiac and respiratory assessment, medication management, pain management, high risk ante partum care, postpartum care, high risk infant, post NICU discharge care
Therapists
Assessment and treatment by Physical Therapists, Occupational Therapists, and Speech-Language Pathologists for adults and children.
Medical Social Workers (MSW)
Our MSW provide case management, grief counseling, coordination of community resources, and assistance with social/emotional effects, as well as coping with acute and chronic illness in the home setting.
Registered Dietitians (RD)
Our RD provide counseling services to individuals who need professional dietary assessment and guidance to properly manage an illness or disability.
Home Health Aides/ CNAs
Our Home Health Aides provide assistance with daily living activities and personal care. They are also trained to provide assistance to high risk pregnant patients while on bedrest.
